Your Hospital Journey to Home at Mercy University Hospital
The ‘Your Hospital Journey to Home’ campaign was launched at the Mercy University Hospital in 2018. This is a hospital-wide quality improvement initiative to ensure early discharge planning and support for patients. The initiative is designed to heighten staff awareness of the importance of patient discharge planning. Patients are asked four questions early in the their hospital journey to obtain information necessary to get them on the correct path to discharge.
Enhancing information and processes at discharge is an area that was identified for improvement, based on the findings of the National Patient Experience Survey 2017. Patients and staff were invited to give their feedback on the initiative. The image on the left shows the form used to get patient and staff feedback on the questions asked.
The hospital plans to monitor patient experience of discharge on an ongoing basis, by surveying patients over the phone, following discharge.
